All information must be entered in full. Your appointment is not confirmed until you have received a confirmation email from us with pre-surgery guidelines.
Surgery will be held at Main Street Vet – 1311 Eatonton Hwy Madison GA 30650 – Sunday November 11th 2018
All pets without a valid rabies tag and certificate will receive one at the time of surgery. Rabies vaccines are $12.
All pets not currently microchipped at the time of surgery will receive a complimentary microchip with free lifetime registration.
