Chips and Snips Registration

All information must be entered in full. Your appointment is not confirmed until you have received a confirmation email from us with pre-surgery guidelines and instructions. 

Surgery will be held at Main Street Vet – 1311 Eatonton Hwy Madison GA 30650 – Sunday November 11th.

If you are not requesting a rabies vaccination at this time, proof of a current rabies vaccination (signed rabies certificate and tag) will be required at check in. If your pet does not have proof of a current rabies vaccination you will need to pay $12 in cash only at check in. All pets are required to have a valid rabies tag and certificate.

Proof of current registered microchip will be required. All pets not currently microchipped at the time of surgery will receive a complimentary microchip with free lifetime registration.

Microchip Only? Please bring your pet to Main Street Vet in Madison, GA on Sunday November 11th between 10 AM & 2 PM to receive a FREE Microchip with FREE Lifetime Registration.
